| ESI Process |
|
Employee State Insurance is a social benefit
scheme which provides medical benefit to employee and his dependent.
You can take medical Treatment in any of the
ESI Dispensary (For list of ESI Dispensary/hospitals you can
log on to ESI website of your State. You can take treatment for
major Diseases in other hospitals also provided doctor in ESI
Hospital Refers you there.
An employee can avail ESI Facility only when his/her
Gross Salary is 10,000 or Below 10,000. In Which Employee pay only
1.75% of his Gross and rest of i.e. 4.75% is paid by the employer.
Procedures to be followed in ESI is as
under:
1. Form No.1 ( can download the same
from forms section)filled up by Employee at the time of Joining along
with 2 Photographs, |

| 2. Form Submitted in ESI Local Office
Along with Form-3 by employer |

| 3. TIC (Temporary Identity Card) along
with Dispensary Issued by the Local Office |
|
| 4. TIC (Temporary Identity Card) is Valid For treatment
for 13 Weeks in allotted Dispensary |
|
|
| 5. After 13 Weeks Employee has to Fill
PIC (Photo Identity Card) Form along with 2 photographs of employee
along with his dependent in the
same photographs is to be attached. In Case of dependent are not
in the photographs they will be denied Treatment. |

| 6. Dependents includes family members
who are directly dependent on employee Like Father, Mother, Spouse
and Children. |

| 7. After attesting the same shall send
to ESI local Office |
|
| 8. Permanent ESI Card Issued By the
ESI Local Office generally within 15 days |
|
| 9. Half yearly return to be filled
in Form 5 by the employer along with 6 monthly challans. |
|
| 10. The employee can avail ESI benefit only
when his full contribution is deposited, Employer has filed proper
returns. |
